- Evaluation of Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
- Detailed intake of medical, social, functional, and family information.
- Identification of current providers and services.
- Medication review, including OTC drugs, prescription medications, and supplements.
- Comprehensive Care Plan tailored to the individual and family.


Basic Care Management
For those needing intermittent in-home Care Management
- Billed Hourly.
- Home visits provided after the Comprehensive Care Assessment.
Includes:
- Documentation and care plan updates.
- Coordination among multiple Care Providers.
- Appointment scheduling.
- Medication Management.
- Discharge planning and Transition support.
- Crisis Intervention (Falls, sudden decline, caregiver burnout.
- Recurring phone calls and emails with providers/family.
- Advocacy and problem-solving.
*In-home Safety Assessments
A home should feel safe, but many older adults face hidden dangers in their daily environments. Changes in balance, strength, and vision can make hazards like loose rugs, poor lighting, slippery bathrooms, and missing handrails more perilous than they seem. Falls are the leading cause of injury among those 65 and older.
Our Certified Aging-in-Place Specialist performs a detailed, room-by-room evaluation to uncover hidden risks and suggest practical solutions - such as improved lighting, removal of tripping hazards, installation of grab bars, durable medical equipment recommendations, and installation of sturdy handrails.

Promotes safer movement, increases confidence, and helps maintain independence at home.

Basic Care Oversight Package
Optimal for stable situations needing basic care guidance and support.
- Billed monthly/ 3-month minimum.
- Up to 8 hours/month of Priority Basic Care Management.
- Additional hours billed at standard rate.
Includes:
- Scheduled In-home visit(s) and observation.
- Care plan monitoring and updates.
- Ongoing management and documentation of current medications.
- Coordination with healthcare providers and agencies.
- Appointment support and follow-up.
- Email/phone support during business hours.
- Monthly written summary and personalized recommendations.
Active Care Coordination Package
Ideal for ongoing, moderate-complexity care.
- Billed monthly/3-month minimum.
- Up to 20 hours/month of Priority Basic Care Management.
- Additional hours billed at standard rate.
Includes:
- Scheduled In-home visit(s) and observation.
- Care plan monitoring and updates.
- Ongoing management and documentation of current medications.
- Coordination with healthcare providers and agencies .
- Appointment support and coordination.
- Email/phone support during business hours.
- Monthly written summary and personalized recommendations.


Comprehensive Care and Advocacy Package
For complex medical, cognitive, or family situations.
- Billed Monthly/3-month minimum.
- Up to 40 hours/month of Priority Care Management.
- Additional hours billed at standard rate.
Includes:
- Frequent scheduled in-home visits and wellness checks.
- On-call support during extended hours (optional add-on).
- Intensive care coordination and advocacy.
- Medication management.
- Hospital, rehab, and transition support.
- Priority response time.
- Weekly family conference for updates.
