In-home Safety Assessments

A home should feel safe, but many impaired and older adults face hidden dangers in their daily environments. Changes in balance, strength, and vision can make hazards like loose rugs, poor lighting, slippery bathrooms, and missing handrails more perilous than they seem. Falls are the leading cause of injury among those 65 and older. 

Our Certified Aging-in-Place Specialist performs a detailed, room-by-room evaluation to uncover hidden risks and suggest practical solutions - such as improved lighting, removal of tripping hazards, installation of grab bars, durable medical equipment recommendations, and installation of sturdy handrails.

Comprehensive 
Care Assessment

An in-home visit by an RN Case Manager to assess, evaluate and document an individuals Medical, Functional and Psycho-social status. The result is a detailed, individualized Plan of Care to help families make informed decisions and gain clarity, confidence, and peace of mind. 
Includes:
  • In-home visit, observation, and Home Safety Assessment.
  • Comprehensive health history review and evaluation of care needs, risks and strengths.
  • Evaluation of Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
  • Detailed intake of medical, social, functional, and family information. 
  • Identification of current providers and services.
  • Medication review, including OTC drugs, prescription medications, and supplements.
  • Comprehensive Plan of Care tailored to the individual and family.
One-time fee: $450

RN Active 
Care Management Services

Short-term, long-term or episodic-based Professional RN Care Management Services tailored to the Client’s needs. 
Includes:
  • Scheduled In-home visit(s) and observation.
  • Care plan monitoring and updates.
  • Ongoing management and documentation of current medications.
  • Coordination with healthcare providers and agencies .
  • Appointment support and coordination.
  • Email/phone support during business hours.
  • Monthly written summary and personalized recommendations.
Hourly fee billed in 15-minute increments.

Normal working hours 8 a.m. to 5 p.m. M-F:
$125/ hour 
After hour Emergency Care and Holidays:
$175/ hour

Care Management Readiness Program

Designed for those who do not require daily, weekly, or monthly Active Care Management Services, but anticipate the need and wish to be prepared. 
  • Keeps Assessments, Plans of Care, Medications, Authorizations, and other documentation current
  • Care givers provide status updates to FCMS
  • Includes a quarterly FMCS check-in via tele-health
  • Does not include Active RN Care Management hours
  • Monthly fee is deducted from the first hour of  Active RN Care Management should it be needed.

Bentley is our H.A.B.I.T.  Program Sponsored Therapy Dog. 

A therapy dog is a specially trained companion who works alongside a healthcare professional to provide comfort, reduce stress, and encourage meaningful interaction. 

Research has shown that animal-assisted therapy can offer meaningful physical and emotional benefits. Spending time with a therapy dog may help reduce anxiety, lower stress, encourage conversation and social interaction, and provide comfort during difficult or uncertain times. 

For many individuals, a gentle nudge, a warm greeting, or simply having a calm companion nearby can make healthcare visits feel less intimidating and more personal.
